Three Letters, One Name Each

For the attorney, the letter is from the state bar. A motion was filed last month with three case citations. Two of those cases do not exist. The bar wants a response within fourteen days.

For the physician, it is from the medical board. A discharge summary went into the patient record with a wrong medication history. A subsequent provider relied on it. The board has questions.

For the financial advisor, it is from FINRA. An investment recommendation went out to a client whose risk profile did not match it. The suitability standard is the standard. The inquiry is open.

Three professions, three envelopes, one shared feature. Each letter names a single human being. Not the firm. Not the hospital system. Not the broker-dealer. Not the AI vendor whose tool drafted the brief, the note, the recommendation. Not the procurement committee that approved the tool. Not the policy that said "always verify AI output."

Just one name, with one license, at one mailbox. That is the accountability gap.

The Gap That Shows Up When the Chain Holds

Earlier in this series I named the attribution gap: what happens at 4pm when nobody can tell who actually did what. The accountability gap is a different problem. It appears when the chain holds.

When the chain holds, it terminates exactly where the regulators designed it to terminate. On a person with a license. The firm absorbs the reputational fallout, manages the settlement, answers the headlines. The professional owns the career. The firm recovers. The license is harder to get back.

This is why "who pays" is worth saying out loud. The answer was set long before anyone deployed AI. It was set by statute, by license, by contract, by the regulator's playbook. AI did not move the endpoint of the chain. It only made the chain longer and harder to see.

A Behavior Nobody Measured

PAICE (People + AI Collaboration Effectiveness) measures AI risk across five dimensions: Performance, Accountability, Integrity, Collaboration, and Evolution. One of them scores lowest, and it is not close. Accountability. It also carries the most weight in the score, 30 percent, because it is the dimension that matters most when the letter arrives.

Accountability is not a value people lack. It is a behavior they were never measured on. Training is not capability. Usage is not quality. Execution gets audited. Behavior underneath the execution does not.

The people most exposed are not the careless ones. They are the confident ones. The AI-fluent operators whose work looks clean, because clean output is exactly when nobody thinks to check the verification habit underneath it. The output looked right, so nobody looked. That is the failure mode that produces the letter.

The Question Under Every Gap

Who pays when the verification did not happen?

That question already has an answer, and you do not get to write it. It is fixed by license and statute and contract. The one thing inside your control is whether you can prove what your people actually did before the answer arrives in the mail.
